Notice of destruction of personally identifiable information
/According to the Greene-Sullivan Cooperative policy, all personally identifiable information related to evaluations and/or special education programming that has been collected and maintained concerning each student must be destroyed within three (3) years after the data is no longer needed for educational purposes.
The following types of personally identifiable information of students who exited the special education programs during the 2021-2022 school year and before will be destroyed.
Individual Education Programs
Individual Tests administered by the Cooperative
Parental Permission Forms
Other Special Education Records
Parents and/or students (18 years of age or older) have the right to examine and copy the information prior to its destruction. The Special Education Office is open Monday through Friday and is located at 77 A St. NE, Linton, IN 47441. You can call the office at 812-847-8497.
Records will be destroyed after August 1, 2025.
